One of the traditions for the WA Yearbook is a tribute to your child including a picture of him/her as a child.  There are three different sizes that you can purchase for your child's yearbook. Please be sure to follow the specifications that are outlined below.

Here are the procedures to follow. Please always include your child's name for our records.  This includes all documentation, pictures sent, etc. 

1. Print and complete PDA Form. Send to the below address along with payment.

2. Type your tribute.  Be sure to check for spelling and grammatical errors.  We will not be responsible for typos. Email your completed tribute to us (word documents are preferred) to the yearbook advisor and be sure to include your child's name in the subject area.

3. Scan the photo that you intend to use and email the digital photo (.jpg format 300 dpi), as well to the advisor with your typed tribute document. If you do not have access to a scanner, please send the photo to us with your child's name written on the back. We will mail the photo back to you. Please include a self addressed stamped envelope if possible.

4. Send your payment (checks written to WA Yearbook please) to the school (see address below). If you prefer to drop the entire tribute off (pictures, payment and tribute text) that is also an option. Sorry we do not accept credit cards.
